RESUMO

Total saikosaponins (TSS) form a group of chemically and biologically active components that can be extracted from Bupleurum, with reported antidepressive, anti-inflammatory, antiviral, antiendotoxin, antitumor, anti-pulmonary fibrosis and anti-gastric ulcer effects. Bupleurum or TSS is frequently utilized in clinical practice alongside other medications (such as entecavir, lamivudine, compound paracetamol and amantadine hydrochloride capsules), leading to an increased risk of drug-drug interactions. The cytochrome P450 (CYP) family serves a critical role in the metabolism of numerous essential drugs (such as tamoxifen, ibuprofen and phenytoin), where the majority of drug interactions involve CYP-mediated metabolism. It is therefore essential to understand the effects of key components of Bupleurum on CYPs when administering combination therapies containing TSS or Bupleurum. The present study aimed to investigate the effects of TSS on the mRNA and protein expression of CYP3A4 and CYP1A2 in HepaRG cells. The effects of TSS on the survival of HepaRG cells was investigated using the Cell Counting Kit-8 (CCK-8) method. Reverse transcription-quantitative polymerase chain reaction (RT-qPCR) and western blot (WB) analysis were used to assess the effects of different concentrations of TSS (0, 5, 10 and 15 µg/ml) on CYP3A4 and CYP1A2 mRNA and protein expression in HepaRG cells. Based on the CCK-8 assay results, it was observed that the cell viability remained above 80% when treated with 1, 5, 10 and 15 µg/ml TSS. Although there was a statistically significant reduced cell viability at TSS concentrations of 10 and 15 µg/ml compared with the control group, the findings indicated that TSS did not exhibit notable cytotoxic effects at these concentrations. Furthermore, RT-qPCR results revealed that compared with those in the control group, TSS at concentrations of 10 and 15 µg/ml reduced CYP3A4 mRNA expression but increased CYP1A2 mRNA expression in HepaRG cells at concentrations of 15 µg/ml. WB analysis found that TSS at concentrations of 10 and 15 µg/ml downregulated CYP3A4 protein expression in HepaRG cells while increasing CYP1A2 protein expression at concentrations of 15 µg/ml. Results in the present study suggest that TSS can inhibit CYP3A4 mRNA and protein expression, but exerts opposite effects on their CYP1A2 counterparts. These findings suggest that it is necessary to consider drug interactions between clinical preparations containing TSS or Bupleurum and drugs metabolized by CYP3A4 and CYP1A2 to avoid potential adverse drug reactions in clinical practice.

In order to promote the sustainable economic development, it is critical employ the digital economy to solve the mismatch dilemma of land and marine factors in coastal areas. It analyzed the influencing mechanisms between the digital economy, land and labor factor mismatch and coastal economic sustainable development using network development and new economic growth theories. The intermediary and regulating effect models were used for empirical tests using panel data from 11 Chinese coastal provinces (city or district) between 2009 and 2018. Results found that: (1) Digital economy promoted the sustainable development of land and marine binary economies in coastal areas; (2) Digital economy improved the factor mismatch of land and marine binary economies, which further affected the sustainable economic development; (3) Market integration is conducive to alleviating land and marine factor mismatch and strengthening the optimization effect of the digital economy on the factor mismatch. This research provides a new perspective for clarifying the mechanism of the digital economy on sustainable economic development, as well as a reference for the realization of rational allocation of factor resources and sustainable economic development by taking factor mismatch of land and marine binary economies and market integration as the intermediary variables and regulatory variables.

BACKGROUND: Several economic studies have assessed the cost-effectiveness of programmed cell death protein-1 (PD-1) inhibitors compared to second-line chemotherapy in treating esophageal squamous cell carcinoma (ESCC). However, there is a lack of economic comparisons among the different PD-1 inhibitors. AIM: This study aimed to assess the cost-effectiveness of PD-1 inhibitors (nivolumab, pembrolizumab, camrelizumab, and tislelizumab) in second-line treatment for advanced or metastatic ESCC within the Chinese healthcare system. METHOD: The clinical trials were systematically retrieved from PubMed, Embase, Web of Science, and the Cochrane Library. We established a fractional polynomials model to conduct a network meta-analysis, enabling the calculation of hazard ratios and expected survival rates. Economic outcomes were estimated using a partitioned survival model. The costs and utilities were gathered from published sources. The threshold for willingness-to-pay (WTP) for a quality-adjusted life year (QALY) was set at three times China's per capita gross domestic product in 2022. Sensitivity analyses (SA) were performed to address uncertainties in the model. RESULTS: Four phase III randomized controlled trials were included, evaluating the cost-effectiveness of four PD-1 inhibitors, camrelizumab, nivolumab, tislelizumab, and pembrolizumab, compared to chemotherapy for the second-line treatment of advanced or metastatic ESCC. For camrelizumab, nivolumab, tislelizumab, and pembrolizumab, the corresponding incremental cost-effectiveness ratios were $27,375.43/QALY, $205,312.19/QALY, $9,266.73/QALY, and $220,368.10/QALY, respectively. The SA results indicated the robustness of the base analysis findings. CONCLUSION: From the Chinese healthcare system, under the WTP of $38,253.48/QALY, tislelizumab is a cost-effective treatment option for the second-line treatment of advanced or metastatic ESCC.

Highland barley flour-based coating batter has rarely been reported, although highland barley flour is promising due to its high ß-glucan and amylose content. In this study, highland barley flour was used to substitute 40% to 80% of wheat flour to form a highland barely-wheat composite flour used in the coating batter. The characteristics of the highland barley-wheat composite flour and its effect on the properties of coating batter and deep-fried meat were analyzed. Results showed that the composite flour significantly improved water holding capacity, oil absorbing capacity, and water solubility index. In contrast, no significant change was observed in the water absorption index or swelling power. The incorporation of highland barley flour significantly changed the pasting properties of the composite flour. Compared with the wheat flour, the viscosity and the pickup of the coating batter made with composite flour increased from 4905 Pa·s and 0.53% to more than 12,252 Pa·s and 0.63%, respectively, and its water mobility decreased. These changes were closely related to the substitution rate of highland barley flour. The composite flour significantly increased the moisture content from 27.73% to more than 33.03% and decreased the oil content of the crust from 19.15% to lower than 16.44%, respectively. It decreased L* and increased a* of the crust and decreased the hardness, adhesiveness, and springiness of the deep-fried meat. A spongy inner structure with a flatter surface was formed in all composite flour-based crusts, and the substitution rate influenced the flatness of the crust. Thus, highland barley flour could be used for batter preparation with partial substitution, enhancing the quality of deep-fried meat and acting as an oil barrier-forming ingredient for fried batter foods.

To develop teff-based food products with acceptable quality, the composition, structure, and properties of teff protein fractions should be better understood. In this study, teff proteins were extracted, and their protein composition, structure, and properties were calculated, analyzed, and compared with those of wheat gliadin and glutenin. Results showed that teff flour contained 9.07% protein, with prolamin as its main protein fraction. The isoelectric points of albumin, globulin, prolamin, and glutelin were at pH 3.6, 3.0, 4.4, and 3.4, respectively. Teff prolamin and glutelin showed a significant difference in amino acids and free energy of hydration compared to wheat gliadins and glutenins. The protein chain length of teff prolamins was smaller than that of wheat gliadins, and teff glutelins lacked high molecular weight glutelin subunits. Teff prolamin had the highest α-helices content (27.08%), whereas no random coils were determined, which is different from wheat gliadin. Teff glutelin had a lower content of ß-turn than wheat glutenin, and no α-helices were determined in it. Teff prolamin and glutelin had lower disulfide bond content and surface hydrophobicity. Teff prolamin had significantly higher thermal stability than wheat gliadin, whereas the thermal stability of teff glutelin was significantly lower than that of wheat glutenin.

OBJECTIVE: To evaluate the risk factors for acute cerebral infarction(ACI) in patients with non-variceal upper gastrointestinal bleeding(NVUGIB), and construct a model for predicting ACI in NVUGIB patients. METHODS: A model for predicting ACI induced by NVUGIB was established on the basis of a retrospective study that involved 1282 patients who were diagnosed with NVUGIB in the emergency department and Gastroenterology Department of Nanchang University Affiliated Ganzhou Hospital from January 2019 to December 2021. Receiver operating characteristic (ROC) curves were drawn to evaluate the sensitivity and specificity of the model and CHA2DS2-VASc score to predict ACI. Delong's test was used to compare AUCs of the present score and the CHA2DS2-VASc score. RESULTS: There were 1282 patients enrolled in the study, including 69 in the ACI group and 1213 in the non-ACI group. Multivariate analysis revealed that hypertension, diabetes, red blood cell (RBC) transfusion, mechanical ventilation, D-dimer, rate pressure product (RPP), somatostatin and mean platelet volume (MPV) were factors associated with ACI induced by NVUGIB. A model based on the eight factors was established, Logit(P)= 0.265 + 1.382 × 1 + 1.120 × 2 + 1.769 × 3 + 0.839 × 4-1.549 × 5-0.361 × 6 + 0.045 × 7 + 1.158 × 8(or 1.069 ×9) (X1, hypertension=1; X2, diabetes=1; X3, RBC transfusion=1; X4, mechanical ventilation=1; X5, somatostatin=1; X6, MPV(fL); X7, D-dimer(ng/l); X8, low RPP= 1; X9, high RPP = 2). The area under ROC curve of the model was 0.873, the sensitivity and specificity were 0.768 and 0.887, respectively. The area under ROC curve of CHA2DS2-VASc score was 0.792, the sensitivity and specificity were 0.728 and 0.716, respectively. Delong's test showed the area under ROC curve of the present study was significantly larger than that of CHA2DS2-VASc score. CONCLUSIONS: Hypertension, diabetes, RBC transfusion, mechanical ventilation, D-dimer, RPP, somatostatin and MPV were factors associated with ACI induced by NVUGIB. A model constructed based on these factors showed excellent prediction of ACI, and was superior to CHA2DS2-VASc score. However, this needs to be further validated by multi-center study with a larger sample size.

BACKGROUND: To investigate the risk factors for gastrointestinal bleeding in patients with cerebral infarction after dual antiplatelet therapy. METHODS: Cerebral infarction patients who received dual antiplatelet therapy during January 2019 and December 2021 in Nanchang University Affiliated Ganzhou Hospital were included. Patients were divided into a bleeding group and a nonbleeding group. Propensity score matching was used to match the data between the two groups. The risk factors for cerebral infarction with gastrointestinal bleeding after receiving dual antiplatelet therapy were analyzed by conditional logistic regression. RESULTS: There were 2370 cerebral infarction patients who received dual antiplatelet therapy included in the study. There were significant differences between the bleeding group and the nonbleeding group in terms of sex, age, smoking, drinking, hypertension, coronary heart disease, diabetes and peptic ulcer before matching. After matching, 85 patients were included in the bleeding group and nonbleeding group, and there was no significant difference between the two groups in terms of sex, age, smoking, drinking, previous cerebral infarction, hypertension, coronary heart disease, diabetes, gout or peptic ulcer. Conditional logistic regression analysis showed that long-term use of aspirin and severity of cerebral infarction were risk factors for gastrointestinal bleeding in cerebral infarction patients receiving dual antiplatelet therapy, whereas the use of PPI was a protective factor against gastrointestinal bleeding. CONCLUSIONS: Long-term use of aspirin and severity of cerebral infarction are risk factors for gastrointestinal bleeding in cerebral infarction patients receiving dual antiplatelet therapy. The use of PPIs could reduce the risk of gastrointestinal bleeding.

As the primary site for the biotransformation of drugs, the liver is the most focused on organ type in pharmaceutical research. However, despite being widely used in pharmaceutical research, animal models have inherent species differences, while two-dimensional (2D) liver cell monocultures or co-cultures and three-dimensional (3D) liver cell monoculture in vitro liver models do not sufficiently represent the complexity of the human liver's structure and function, making the evaluation results from these tools less reliable. Therefore, there is a pressing need to develop more representative in vitro liver models for pharmaceutical research. Fortunately, an exciting new development in recent years has been the emergence of 3D liver cell co-culture models. These models hold great promise as in vitro pharmaceutical research tools, because they can reproduce liver structure and function more practically. This review begins by explaining the structure and main cell composition of the liver, before introducing the potential advantages of 3D cell co-culture liver models for pharmaceutical research. We also discuss the main sources of hepatocytes and the 3D cell co-culture methods used in constructing these models. In addition, we explore the applications of 3D cell co-culture liver models with different functional states and suggest prospects for their further development.

Small yellow croaker (Larimichthys polyactis), a commercially essential fish commonly caught in China and South Korea, is now facing a severe decline in resources. The recruitment and surplus of L. polyactis depend selecting a suitable marine environment for overwintering. However, the international overwintering migration habit of L. polyactis limits the investigation of its overwintering environment preferences and suitable grounds. In this study, based on the distribution data of L. polyactis in the southern Yellow Sea in winter from 2010 to 2019 and ocean remote sensing data such as sea bottom temperature (SBT), sea bottom salinity, chlorophyll-a concentration and water depth (Depth), we used the maximum entropy (MaxEnt) and the genetic algorithm for rule-set production (GARP) models to investigate the overwintering grounds of the southern Yellow Sea stock (SYS). The jackknife test was used to assess the importance of various environmental factors. For modelling the overwintering ground distribution of SYS, the area under the curve values of both models ​were higher than 0.9. The overwintering ground was at 32°10' N-33°48' N, 122°30' E-125°00' E. The direction of its distribution was consistent with the Yellow Sea Warm Current in the southern Yellow Sea during the winter. Compared with the suitable overwintering area during 2010-2014, the highly appropriate overwintering area for SYS to overwinter decreased significantly during 2015-2019, showing a trend of moving to the east and north, related to the increase in fishing pressure and strengthening of the Yellow Sea Warm Current in recent years. Depth was the most significant factor for SYS overwintering, followed by SBT. The overwintering ground was at a depth of 40-65 m during the two periods. Additionally, the suitability of overwintering grounds in the coastal waters of south-western South Korea has gradually increased. This study provides a scientific basis for formulating effective strategies to manage L. polyactis resources under the China-South Korea Fisheries Agreement.

A theoretical mechanism was analyzed from the micro perspective of the enterprise to explore how information accessibility moderates the effect of accounting manipulation on the sustainable development of digital enterprises. Using data from 1200 listing digital enterprises in China and the DEA-Malmquist index method, the efficiency value of digital enterprises in 2007-2021 was estimated to represent the index of sustainable development of digital enterprises. The accounting manipulation was detected using the panel PSM-DID method based on the Administrative Measures for the Recognition of High-tech Enterprise's policy. The information accessibility value was estimated based on the MDA method. Empirical studies were conducted using text analysis, the panel PSM-DID method, and the double moderating effect model. The results showed that: (1) Accounting manipulation had a negative impact on the sustainable development of "true" digital enterprises and the "fake" digital enterprises. (2) Information accessibility directly and positively enhanced the technological progress and scale efficiency of digital enterprises, and its moderating effect was heterogeneous, with a significant moderating effect on the "true" digital enterprises and a negative effect on the "fake" ones.

The optimal number of examined lymph nodes (ELNs) for gastric signet ring cell carcinoma recommended by National Comprehensive Cancer Network guidelines remains unclear. This study aimed to determine the optimal number of ELNs and investigate its prognostic significance. In this study, we included 1723 patients diagnosed with gastric signet ring cell carcinoma in the Surveillance, Epidemiology, and End Results database. X-tile software was used to calculate the cutoff value of ELNs, and the optimal number of ELNs was found to be 32 for adequate nodal staging. In addition, we performed propensity score matching (PSM) analysis to compare the 1-, 3-, and 5-year survival rates; 1-, 3-, and 5-year survival rates for total examined lymph nodes (ELNs < 32 vs. ELNs ≥ 32) were 71.7% vs. 80.1% (p = 0.008), 41.8% vs. 51.2% (p = 0.009), and 27% vs. 30.2% (p = 0.032), respectively. Furthermore, a predictive model based on 32 ELNs was developed and displayed as a nomogram. The model showed good predictive ability performance, and machine learning validated the importance of the optimal number of ELNs in predicting prognosis.

BACKGROUND: At present, the quantifiable pressure distribution at the interface between the socket and stump is seldom applied in the design and fabrication of the socket. OBJECTIVES: This study aimed to optimize the socket based on the interface pressure of residual limb-socket, thereby avoiding excessive local load on the residual limb, reducing the load on the pressure-sensitive (PS) regions and making the limb more evenly loaded. METHODS: The residual limb was divided into the main load-bearing regions, the pressure-tolerant regions, and the PS regions according to the carrying capacity at its different regions. Based on these bearing regions, a mathematical function was developed, which applied modifications/adjustments to the socket design in a Computer Aided Design (CAD) environment by using the adjustment function. Besides, three adjusted sockets were produced by using selective laser sintering 3D printing technology. RESULTS: The wearing of the 3D-adjusted printed sockets reduced the contact interface pressures in the distal tibial region and the fibular head region by 85.6% and 84.4%, respectively. In addition, the walking distance of the subject was increased by 18.34%, and the overall pressure distribution on the stump became more uniform. CONCLUSIONS: The pressures in the original overpressure regions and the PS regions could reduce, whereas the pressure in the low-load regions of main load-bearing or pressure-tolerant regions could increase by modifying the socket with the pressure adjustment function. At the same time, the pressure among different regions was more uniform except for the sensitive regions.

A rhodium(III)-catalyzed regioselective C2-alkenylation of indoles for the construction of α-CF3 substituted enamines has been developed, which utilizes CF3-imidoyl sulfoxonium ylides (TFISYs) as alkenylating agents for the first time. A wide array of indolyl- and trifluoromethyl-decorated enamine derivatives have been assembled in moderate to good yields.

A key primary method for creating a carbon cycle and carbon neutrality is the catalytic hydrogenation of CO2 into high value-added chemicals or fuels. In this work, ZnGaOx oxides were prepared by parallel co-precipitation and physically mixed with SAPO-34 molecular sieves prepared by hydrothermal synthesis to produce ZnGaOx /SAPO-34 bifunctional catalysts, which were evaluated for the catalytic synthesis of lower olefins (C2 = -C4 = ) from carbon dioxide hydrogenation. It was demonstrated that the reaction process requires oxygen defect activation, synergistic hydrogenation, and CO2 alkaline adsorption of ZnGaOx . The spinel structure of ZnGaOx has more abundant oxygen defects and alkaline adsorption sites than the ZnGaOx solid solution, which effectively enhances the catalytic performance. The CO2 conversion was 28.52%, the selectivity of C2 = -C4 = in hydrocarbons reached 70.01%, and the single-pass yield of C2 = -C4 = was 10.95% at 370 °C, 3.0 MPa, and 4800 mL/gcat /h.

A ruthenium-catalyzed peri-selective C-H activation and annulation of 1-naphthols with CF3-substituted imidoyl sulfoxonium ylides that uses hydroxyl as a weakly coordinating directing group is disclosed. The strategy provides a facile and practical route to diverse trifluoromethyl-containing 2,3-dihydrobenzo[de]chromen-2-amines with high efficiency. Notable advantages of this protocol include readily available materials, excellent regioselectivity, good functional group compatibility, and scalability.

The graphene modified asphalt used in this study was prepared based on a highway project in Gansu Province. In this paper, the high temperature rutting resistance, low temperature cracking resistance, and water stability of SMA-13 asphalt mixture with asphalt (AH-70), SBS modified asphalt and graphene rubber composite modified asphalt were tested and analyzed comparatively by the rutting test, Schellenberg binder drainage test, Cantabro test, freeze-thaw splitting test, and beam bending test. The results showed that the graphene modifier improved the asphalt's ductility and softening point significantly, and 0.4g graphene content was the threshold and its corresponding mixture performance index. In the other tests under the same conditions, the high temperature and water stability of SMA-13 mixtures of graphene rubber modified asphalt were the best, followed by SBS modified asphalt mixture, and matrix asphalt mixture. Compared with wood fiber, graphene modifier had no significant effect on SMA-13 mixtures' low temperature performance. The use of graphene modifiers can enhance the adhesion between asphalt and aggregate and its asphalt has good consistency and viscosity. When compared with matrix asphalt and SBS modified SMA-13 mixtures, the water and high temperature stability of graphene modified asphalt mixture is better.

PURPOSE: In this study, we analyzed the clinical distribution and drug resistance of carbapenem-resistant Klebsiella pneumoniae (CRKP) strains, the minimum inhibitory concentrations (MIC), MIC50 and MIC90, and geographical distribution in Hebei Province, China. We aimed to provide epidemiological research data, formulate appropriate combined treatment schemes, reasonably select antibiotics, and standardize nosocomial infection control schemes. PATIENTS AND METHODS: A total of 6328 strains of CRKP were collected from 2017 to 2019. The MIC was determined for the drug sensitivity test, and the experimental data were statistically analyzed using WHONET5.6. RESULTS: The detection rate of CRKP increased annually from 13.4% in 2017 to 14.5% in 2018, and 14.6% in 2019. The ratio of males to females was approximately 2:1; 53.6% were elderly, 39% were adults, 4.8% were minors, and 2.5% were newborns. The specimens collected were mainly sputum (70.9%). The resistance rate of CRKP to carbapenems and other ß-lactam antibiotics was found to be increasing, with resistance rates generally greater than 90%. The resistance rate to aminoglycoside antibiotics decreased yearly to approximately 50%, and the resistance rate to quinolones remained unchanged at approximately 80%. From 2017 to 2019, the resistance rate of CRKP in Hebei Province to various antibiotics was high, and the resistance rate to ß-lactam antibiotics increased each year. CONCLUSION: The situation of CRKP resistance is severe in Hebei Province, China. The resistance rate to most antibiotics is very high and shows an upward trend. Among them, the resistance rate to polymyxin is low; however, few resistant strains do exist. MIC50 and MIC90 are higher than their MICs. It mainly causes lung infection in elderly men. This study is helpful to improve the diagnosis, treatment, and prevention of CRKP infection in our province.

Microbiologically influenced corrosion (MIC) has caused great losses to many industries. This paper aimed to study the corrosion behavior of P. aeruginosa on X65 steel. The corrosion behavior of P. aeruginosa on X65 steel under aerobic and anaerobic conditions was studied by scanning electron microscopy, energy dispersive spectrometer and electrochemical analysis techniques. The results showed that the corrosion rate of X65 steel in bacterial environment is higher than that in sterile environment. In anaerobic environment, the corrosion of P. aeruginosa is mainly secreted acidic metabolites, and alkaline substances are corroded in aerobic environment. In general, the corrosion of X65 steel by P. aeruginosa in aerobic environment is more serious than that in anaerobic environment.

OBJECTIVE: Alterations in the structure and function of intervertebral discs by multifaceted chronic processes can result in intervertebral disc degeneration (IDD). The mechanisms involved in IDD are still unknown. METHODS: We investigated the possible mechanisms underlying IDD using a bioinformatics analysis of publicly available microarray expression datasets and built a circular RNA-microRNA-mRNA (circRNA-miRNA-mRNA) network based on the results. Datasets GSE67566 and GSE116726 were downloaded from the Gene Expression Omnibus (GEO) and analyzed using the limma package in R. The CircInteractome database was used to detect miRNAs related to circRNA, and TargetScan, miRDB, and miRTarBase were used to predict target mRNAs. Key target genes were annotated using Gene Ontology terms. RESULTS: The circRNA hsa-circ-0040039 was found to have the top log fold-change score. Analysis using Metascape showed that the associated genes were enriched mainly in the cell cycle. The Cytoscape plugin MCODE predicted that two members of the RAS oncogene family-RAB1A and RAB1B-and multiple coagulation factor deficiency (MCFD2) may play key roles in IDD. CONCLUSION: Our results suggested that hsa-circ-0040039 and the related network may be potential biomarkers for IDD.
